REGISTRATION
IMPORTANT INFORMATION
PLEASE NOTE:
The schedules for Children’s and Tiny Tots’ Fall and Winter classes are available under ‘Schedules.’ Please make note of the void dates. Please remember to print or save a PDF of your receipt in Step 4. No other receipt or notification will be issued. This receipt is your confirmation of registration.
Payment for classes is by Credit/Debit Card only via PayPal. You DO NOT need a PayPal account to pay (click on 'Don't Have A PayPal Account?’). Your credit card statement will show a charge from "YIPOH." Please, do not dispute this charge.
Always refer to the brochure under ‘Schedules’ to confirm class times.
Prices are pro-rated at registration.
Children must be in the stated age range by the first class of the session. A BIRTH CERTIFICATE or EQUIVALENT IDENTIFICATION may be requested at the first class for each child registered. Anyone registering a child with special needs or food alergies should contact the Children's Committee at ahcc@yipoh.com prior to registering. All issues should also be discussed with the Class Coordinator and the Instructor prior to the first class.
Late Registration:
Late registrations create added work for the Children’s Committee volunteers and instructors. Please make every effort to register online prior to the first class. Thank you. If space is available, late registrations will be accepted at the discretion of the program coordinator. You must still register online. Classes will NOT be pro-rated.
Cancelled Programs:
We reserve the right to cancel classes due to insufficient enrollment, a strike or an election. For these reasons only, a full refund will be issued or classes will be rescheduled.
Refunds:
There will be a $25 administration fee if you withdraw from the class from week 2 to the end of the term. If you withdraw prior to the start of classes, a full refund will be issued.
PLEASE NOTE:
We reserve the right to remove from a program, without refund, any child who is being overly disruptive or unruly, does not follow the instructor's direction or who jeopardizes the safety of the participants or instructors.
WAIVER
IN CONSIDERATION of allowing my minor child/ward to participate in Children’s classes at Armour Heights Community Centre, I AGREE THAT:
1. I am a parent or guardian having full legal responsibility for decisions regarding my minor child/ward.
2. I am familiar with the risks which any participant in this program must acknowledge.
3. I believe that my minor child/ward is physically, emotionally and mentally able to participate in this program.
4. I understand and will instruct my minor child/ward, that all applicable rules for participation must be followed.
5. I will immediately remove my minor child/ward from participation, and notify the instructor , if at any time I sense or observe any unusual hazard or unsafe condition or if I feel that my minor child/ward has experienced any deterioration in his/her physical, emotional or mental fitness for continued participation in the program.
IN CONSIDERATION of allowing my minor child/ward to participate in Tiny Tots classes at Armour Heights Community Centre, I AGREE THAT:
1. I understand that it is my responsibility or my caregiver’s responsibility to protect and supervise my child at all times.
2. I will ensure that my child does not engage in any activities that put him/her at risk of injury.
I UNDERSTAND AND AGREE, ON BEHALF OF MY MINOR CHILD/WARD, MYSELF, MY HEIRS, ASSIGNS, PERSONAL REPRESENTATIVES AND NEXT OF KIN, THAT MY EXECUTION OF THIS DOCUMENT CONSTITUTES:
1. An unqualified ACKNOWLEDGEMENT OF ALL RISKS associated with participation in this program by my minor child/ward even if arising from negligence or equipment failure.
2. A FULL AND FINAL RELEASE AND WAIVER OF LIABILITY of the program organizer and all persons and organizations associated with it and the program including, without limiting the generality of the foregoing, its volunteers, instructors, instructor assistants, directors, employees, other participants, owners and/or lessors of the premises used to conduct the program, (the RELEASEES), of and from with the respect to all injury, disability, death or loss or damage to person or property even if arising from the negligence.
3. An UNDERSTANDING NOT TO SUE THE RELEASEES for any loss, injury, costs or damages of any form or type, howsoever caused or arising, and whether directly or indirectly from the participation of my minor child/ward in the program, and
4. An AGREEMENT TO INDEMNIFY, and to SAVE AND HOLD HARMLESS THE RELEASEES, and each of them, from any litigation expense, legal fees, liability, damage, award or cost, of any form or type whatsoever, they may incur due to any claim made against them or anyone of them whether the claim is based on the negligence of the RELEASEES or otherwise.
5. I UNDERSTAND THAT THE RELEASEES ARE RELYING UPON MY WARRANTIES, ACKNOWLEDGEMENTS, WAIVER AND RELEASE, UNDERTAKINGS AND AGREEMENTS WHEN ACCEPTING MY MINOR CHILD'S/WARD'S PARTICIPATION IN THIS PROGRAM.
6. I UNDERSTAND THAT BY SIGNING THIS DOCUMENT I GIVE UP SUBSTANTIAL LEGAL RIGHTS I AND/OR MY MINOR CHILD/WARD WOULD OTHERWISE HAVE.
7. I CONFIRM THAT I have had sufficient time to read and understand each term in this waiver in its entirety, and have agreed to the terms freely and voluntarily. I understand that this waiver is binding on my Legal Representatives and myself.
I ACKNOWLEDGE THAT BY REGISTERING MY MINOR CHILD/WARD I AM AGREEING TO THE TERMS OF THIS WAIVER.
